- Name
- Address
- Phone number
- Type of vehicle - ensure that the vehicle is the same as the pink card
- Year of vehicle and the VIN
- Insuring company

- Police charges, if applicable
- Name of other drivers and vehicles involved
- Nature of damage
- Details of injuries, if applicable
